Vivo HealthStaff is recruiting for a position in the Tucson, Arizona area. Relocation is provided for the right Candidate.
Job Overview :
Join our healthcare team as a Board-Certified or Board-Eligible Internal Medicine or Family Practice Physician. In this role, you will lead patient care in a collaborative environment, managing complex cases with the support of advanced practice providers, social workers, case managers, and other health professionals. You'll participate in multidisciplinary conferences to focus on high-risk patients. Our facility provides access to advanced technology, including a robust EMR system and a proprietary decision-support tool that consolidates data from various sources into actionable insights to enhance patient care.
Responsibilities :
- Deliver evidence-based primary care, with a focus on senior patients.
- Conduct annual wellness exams for new and existing patients.
- Perform minor office procedures as needed.
- Coordinate patient care with specialists and ancillary providers.
- Ensure patients have advanced care planning, and educate them on health topics such as disease management, medications, healthy lifestyle choices, and end-of-life care.
- Participate in and sometimes lead interdisciplinary team conferences.
- Manage population health for your assigned patient panel.
- Work towards achieving HEDIS and CAHPS goals.
- Strive to provide exceptional patient experiences and accessibility.
- Uphold a commitment to innovation and the continuous improvement of healthcare practices.
- Contribute to quality improvement, peer review, and special projects as required.
- Adhere to HIPAA regulations and maintain the confidentiality of protected health information (PHI).
Supervisory Responsibilities :
Assist in supervising medical assistants and other office staff.May directly supervise advanced practice clinicians.Required Skills and Experience :
Preferably 2 years of experience as an Internal Medicine or Family Practice clinician.1-3 years of managed care experience is preferred.Basic understanding of utilization management and quality management processes.Completion of a U.S. residency program in Internal Medicine or Family Medicine.Board certification or the ability to achieve board certification within 6 months of hire.Possession of a medical license to practice in the state of employment.Substantial knowledge of various medical subspecialties.Clinical confidence and an approachable "bedside manner."Flexibility and a willingness to learn new systems and processes.Strong communication skills.
