Job Description :
We are seeking a compassionate and detail-oriented Care Coordination Specialist to join our team. This role is responsible for ensuring seamless communication between clients, caregivers, and healthcare professionals, and for managing the coordination of care services to meet individual needs efficiently.
Key Responsibilities :
- Act as the primary point of contact for families and caregivers.
- Coordinate and schedule in-home care services based on client needs and availability.
- Monitor service delivery and follow up with clients to ensure satisfaction.
- Collaborate with healthcare providers and internal teams to update care plans.
- Maintain accurate records in the care management system.
- Respond promptly to client inquiries, changes, or emergencies.
- Assist in matching caregivers with clients based on compatibility and skills.
- Support onboarding and training for new caregivers.
Qualifications :
Requirements
Associate’s or bachelor’s degree in administration, health, social work, or related field preferred
1+ year of experience in care coordination, case management, or customer service (preferably in a healthcare or home care setting)
Proficient in Google Worskapce, Zoho CRM, Whatsapp and other administrative tools
Reliable internet connection and ability to work from home
Flexibility to work evenings or weekends occasionally if client needs arise
Demonstrated empathy, discretion, and professionalism
