Certified Medical Assistant

To support proactive care coordination for patients with chronic conditions, the fully remote Certified Medical Assistant (Care Coordinator) will conduct patient outreach, maintain care plans, and ensure documentation compliance while managing a high-volume patient panel. Key responsibilities Support patient enrollment, verify eligibility, and maintain patient engagement Conduct weekly outreach via text and phone to enrolled patients, reinforcing care plans and identifying barriers to adherence Document patient interactions accurately and maintain compliance with billing and HIPAA standards Required qualifications Certified Medical Assistant 1-3 years of healthcare experience Experience in chronic disease or nephrology preferred Strong clinical assessment and documentation skills Intermediate proficiency with MS Office Suite
