In-Home Nurse Practitioner / Physicians

Descripción del trabajoComplete a detailed evaluation of the medical and social assessment of the beneficiary members. Identify and refer members / patients for routine, urgent, and emergent case management. Make recommendations for member's follow-up with their primary care physician. Not required to prescribe or alter the member's current treatment plans. Complete comprehensive assessment face to face in home setting. At least 1 year hands on experience as a Nurse Practitioner / Physician Must be registered with Medicare and / or Medicaid Current, unrestricted license in the state of assignment Must have license / certification : APRN, Certified Nurse Practitioner Knowledge of HCC model, HEDIS and STAR quality measures Effectively communicate with elderly and chronically ill members Proficient in use of computer, tablet, and mobile technology Ability to document within available EMR. About the Role :
We are looking for a Nurse Practitioner / Physicians to join our Clinical team. The Nurse Practitioner / Physician will conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members / patients of primary care physicians, medical groups, and the health plans we serve. The Nurse Practitioner / Physician will perform comprehensive assessments which are compliant with the annual wellness visit in the patient's home.
Location :
The positions are based in Ponce, Puerto Rico. We require the applicant to have reliable transportation to travel to patient's home.
What We Offer
- Independent Contractor
- Flexible schedule (Monday through Sunday 8am to 7pm)
- 1-hour session per visit
- Compensation : $120 per completed visit
